Physical Therapist (PT)

Location: Longevity Physical Therapy (outpatient clinic) in St. Augustine, FL
We partner with St. Augustine Foot, Ankle & Vein.
Schedule:
Monday through Friday, 7:45 AM to 4:45 PM
Flexibility: We are actively growing our Physical Therapy Department. During the early stages of practice growth, the selected candidate may have the flexibility to work a modified schedule, including part-time hours or shorter days, while helping build and expand the program. As patient demand increases, the schedule is anticipated to grow into a full-time caseload.

Position Summary:
We are seeking an experienced, compassionate, energetic, and growth-minded Physical Therapist to join our multidisciplinary practice. The ideal candidate will be passionate about helping patients achieve exceptional outcomes while contributing to the continued growth and success of our Physical Therapy Department.

This position requires a clinician who excels in patient care, is highly organized, confident in their clinical skills, and comfortable building relationships with both patients and referring providers. The ideal candidate is a self-starter who enjoys helping grow a practice, tracking performance metrics, and creating an outstanding patient experience.

Essential Duties and Responsibilities:
Patient Care & Clinical Treatment:
  • Perform comprehensive evaluations and develop individualized treatment plans.
  • Provide physical therapy services for:
    • Post-surgical rehabilitation.
    • Orthopedic and musculoskeletal injuries.
    • Lymphedema management and treatment.
    • Myofascial release therapy.
    • Cupping therapy.
    • Functional rehabilitation and recovery programs.
  • Monitor patient progress and adjust treatment plans as necessary.
  • Educate patients on treatment plans, home exercise programs, and long-term wellness strategies.
  • Deliver exceptional patient-centered care with empathy, compassion, and professionalism.
  • Responsible for discharge planning and creating effective care plans.

Patient Relationship Management:
  • Personally follow up with patients regarding treatment progress and outcomes.
  • Contact patients who miss appointments to encourage continuity of care and rescheduling.
  • Manage the practice's lymphedema recall program, including contacting patients every six months regarding follow-up evaluations and garment replacement needs.
  • Build strong patient relationships that improve compliance, retention, and treatment success.

Practice Growth & Community Outreach:
  • Represent the practice at provider breakfasts, luncheons, networking events, and community outreach opportunities.
  • Educate physicians and healthcare providers about the physical therapy services available within the practice.
  • Develop and maintain strong referral relationships.
  • Refer internal patients to our other departments when appropriate.
  • Participate in initiatives designed to grow patient volume and expand the physical therapy department.
  • Demonstrate enthusiasm and commitment to helping build a thriving rehabilitation program.

Operational Excellence:
  • Complete accurate, compliant, and timely documentation.
  • Maintain efficient note completion while meeting all clinical and regulatory standards.
  • Track patient outcomes, treatment progress, and departmental metrics using Excel and other reporting tools.
  • Monitor and understand department collections and key performance indicators.
  • Maintain organized patient records and treatment plans.
  • Effectively manage schedules and patient flow.
  • Treat multiple patients simultaneously when clinically appropriate and permitted by insurance guidelines.
  • Demonstrate excellent multitasking and time-management skills.

Qualifications:
Education & Licensure:
  • Current Florida Physical Therapist License required.
  • DPT strongly preferred; candidates with MSPT or BSPT degrees and relevant experience will be considered.

Computer Required Experience:
  • Strong proficiency with Microsoft Excel and performance tracking.
  • Demonstrate typing proficiency of 60+ words per minute.

Preferred Qualifications:
  • Certified Lymphedema Therapist (CLT) certification preferred, or significant experience treating lymphedema patients.
  • Experience with pelvic floor rehabilitation and treatment is highly desirable.
  • Certified Pelvic Floor Therapist or equivalent advanced training is a plus.
  • Experience treating pelvic floor dysfunction, urinary incontinence, pelvic pain, postpartum rehabilitation, and related conditions preferred.
